If you are a 'Carer' and look after someone or if you are cared for by someone it is important that you let us know. We may be able to provide you with information to assist you in this role, to offer you support or to direct you to organisations who may be able to provide financial assistance.

Chronic Disease Management Clinics

docsteth1All recent research has shown that conditions that are longstanding and require regular monitoring are best looked after in a clinic setting. The clinics we run are designed to ensure that we provide the most appropriate and up to date care and that you are reviewed regularly.

Patients suffering from the conditions listed below will be called automatically for a review. All clinics take place at our main surgery in Selby.

Asthma & Chronic Obstructive Pulmonary Disease (COPD)

You should attend once a year in the Respiratory Clinic. Clinics are held on Tuesday & Friday mornings, 8.30 - 11.30 am and Friday afternoons, 2.30 - 5.30 pm

Heart/Stroke

You should attend twice a year. Six month reviews are held with a Practice Nurse every Tuesday morning between 8.45 and 11.45. Annual reviews are held every Tuesday morning between 8.30 and 10.50 with either Dr Scott or Dr Merrifield.

Diabetes

You should attend twice a year. Six month reviews are held with a Practice Nurse every Tuesday afternoon, 1.00 - 5.00 pm and Wednesdaty afternoon, 2.30 - 4.30 pm. Annual reviews are held every Thursday morning, 8.30 - 11.30 am

Hypertension (High Blood Pressure)

You should attend every 9 months. Clinics are held on a Tuesday & Thursday morning, 8.30 - 11.30 am. 

Your time at the clinic is to monitor a specific condition ONLY and we ask that you make a separate appointment for any other medical conditions you may have.
