Chronic Disease Management Clinics

All recent research has shown that conditions that are longstanding and require regular monitoring are best looked after in a clinic setting. The clinics we run are designed to ensure that we provide the most appropriate and up to date care and that you are reviewed regularly.

Patients suffering from the conditions listed below will be called automatically for a review. All clinics take place at our main surgery in Selby.

Your time at the clinic is to monitor a specific condition ONLY and we ask that you make a separate appointment for any other medical conditions you may have.

Asthma & Chronic Obstructive Pulmonary Disease (COPD)

You should attend once a year in the Respiratory Clinic. 

Heart/Stroke

You should attend every 6 months. 

Diabetes

You should attend every 6 months. 

Hypertension (High Blood Pressure)

You should attend every 9 months for your blood pressure checked in a BP Clinic

Other Dedicated Clinics

Blood Tests/Phlebotomy

Patients who need to give a sample of blood should attend outpatients at Selby War Memorial Hospital. The clinic operates between 8.30 am & 4.30 pm Monday to Thursday & 8.30 am - 12.00 noon on Friday's.

You must make an appointment by calling 01904 - 724310 between 9.00 am and 12.00 noon.

Breast Screening

North Yorkshire Breast Screening Service provide this service. The service write to all eligible women (those aged between 30 and 70) every 3 years to invite them for screening. The mobile screening unit is based at Selby War Memorial Hospital.

Cervical Smear Tests

These important checks facilitate early detection of cancer of the cervix (neck of the womb). All female patients between the ages of 25 and 65 should be tested every 3 years, or more regularly if advised for certain conditions. Younger patients may also be recommended for testing. Appointments can be made with the nurse; results can take up to 8 weeks to get back from the laboratory.

Child Development Checks

Checks are made on all children during their pre-school years (see below)

• 8 weeks: Child seen by GP (parents need to make this appointment themselves)
• 2 years: Child seen by Health Visitor for developmental assessment

Letters are sent by the practice inviting parents to make an appointment for their child for a 8 week check.

Family Planning and Emergency Contraception

We provide a full contraceptive service in normal surgery hours. Emergency contraception is available from the nurses as well as doctors, and it is important to be seen within 72 hours of any potential contraceptive failure or omission. This service is also available from the Minor Injuries Unit at Selby War Memorial Hospital.

Flu Vaccinations

The Department of Health recommend that certain groups of the population have a seasonal flu vaccination each year. (DoH criteria)

All patients eligible for a flu vaccination will be contacted by letter mid/late September each year. Due to the large number of patients to be vaccinated, dedicated clinics are set up that run from our main surgery in Selby on  Saturday's. These appointments are for FLU VACCINATIONS ONLY. Appointments for flu vaccinations at our branch surgeries of Carlton & Riccall are not usually until later in the year.

Health Checks

Patients aged between 40 and 74 years of age will be invited for a 'NHS Health Check' once every 5 years.

The check will be carried out by a Health Care Assistantand is designed to assess risk of developing Heart Disease, Stroke, KidneyDisease or Diabetes.  If there are anywarning signs we can do something about it. By taking early action, you can improve your health and prevent theonset of these conditions.

The check should take about 20 minutes and is based onstraightforward questions and measurements such as age, sex, family history,height, weight, and blood pressure. There will also be a simple blood test to measure your cholesterol level.

Maternity

Dr Cunningham runs weekly postnatal clinics. The Midwife runs her own antenatal clinic alongside these.

Minor Surgery

Some of the Partners undertake minor surgical procedures, such as joint injections, removal of lumps and moles etc. This is usually done at Selby hospital on a Monday or Wednesday morning. Before any such procedure can be undertaken it is important you see a doctor in surgery first.

Sexual Health Service  

This is a drop in service run alongside the Family Planning Service and provided free and confidential advice, testing and treatment of sexually transmitted infection. The service is run from Raincliffe Street Health Centre, Raincliffe Street, Selby, For more information call 01904 - 725412.

Stop Smoking Groups

Various options of support are available, these include drop in clinics, 1-1 appointments, group courses and specialist help for those in pregnancy who would like to stop smoking.

If you require any vaccinations relating to foreign travel you need to complete a travel questionnairre (see below). A Nurse will review your questionnairre & your existing vaccination history within 1 week of receipt. A nurse will then call you to book an appointment in a Travel Clinic 

Please ensure your questionnairre is returned to us at least 4 weeks prior to your departure date.

We ONLY offer vaccinations that are available on the NHS. Other vaccinations that may be required including Hepatitis B, Rabies, Meningitis, Japanese Encephalitis, Yellow Fever are considered private and can be obtained from the Travel Clinic in York, Tel: 0330 100 4325, www.masta-travel.com.

Immunisations

Children

Immunisations are essential to protect children from serious illness. We run a computerised recall system to remind parent's when their child's vaccinations are due.

 Vaccinations for young children, those 8 weeks of age are given by a GP. All other vaccinations are given by Gill Gibbon, a member of our nursing team.

Immunisation clinics for children run weekly, from our main site in Selby, clinic times are as follows:

• Mondays from 10.30 am - 12.00 noon
• Wednesdays from 2.30 pm - 4.30 pm

Adults

Most vaccinations (including yellow fever) are given by the Practice Nurses. We stock most, but some unusual injections may need to be ordered. Whilst common injections are still available on the NHS, rarer ones are not, and a fee will be charged for them. You will be advised of the cost before the injection is ordered.

Certificates for travel are available and a fee will be charged for completion of these.

Adults are advised to have Tetanus and Polio boosters every 10 years.

Each autumn we run a Flu injection campaign, particularly important for those most at risk; those aged over 65, diabetics, asthmatics, those with chronic heart disease, chest and kidney problems.

Non-NHS Work

We are happy to undertake, by appointment, medical examinations, reports for insurance companies, employers and solicitors. We can carry out fitness medicals for sports, elderly drivers etc.

Unfortunately the NHS does not cover these services and a fee will be charged, likewise, we will charge a fee for private certificates. Fee levels are displayed below and at each of our surgeries. Forms of such a nature will usually be ready for collection 7 working days after receipt although we recommend you contact our main surgery in Selby to ensure your form has been completed.

Payment for all work is required in advance

Due to changes in NHS funding from 1 April 2006 and HM Customs & Revenue revisions from 1 May 2007 some fees are eligible for VAT at the prevailing rate. Fees marked with * are inclusive of VAT at the standard rate, all others are exempt from VAT
