If you require any vaccinations relating to foreign travel you need to complete a travel questionnairre (see below).
Please ensure your questionnairre is returned to us at least 6 weeks prior to your departure date.

We ONLY offer vaccinations that are available on the NHS. Other vaccinations that may be required including Hepatitis B, Rabies, Meningitis, Japanese Encephalitis, Yellow Fever are considered private and can be obtained from the Travel Clinic in York, Tel: 0330 100 4325, www.masta-travel.com.
